Club Member Forms

• MEC Release Form - All club riders must submit to advisor
• MEC Riding Placement Form - All club riders must submit to advisor

IHSA Team Forms -  Submit forms to the Coach by deadlines

IHSA Rule Book 2011 - 2012    All IHSA Team Members Should Be Familiar with the IHSA Rule Book

• Mercer Equestrian IHSA Team Handbook and Form READ, sign and submit to Coach
• IHSA Rider Placement Form - Only new team members need to complete this form-USE A PENCIL, coach will review
• IHSA Zone 5 Region 3 Questionnaire Form All team riders must complete each year - USE A PENCIL- will review
• IHSA Team Individual Membership Form  Login - complete online, print and submit to Coach
• USHJA Collegiate(IHSA) Membership Form and FAQS - Only Hunt Seat Riders need to complete this form
• Lesson Verification Form - Submit to Coach by deadlines

There will be a meeting to review these forms before finalizing them.  If you have shown at recognized AQHA shows or USEF  H/J shows you may need to provide verification of your show record from online databases maintained by AQHA and USEF.

Because IHSA riders range beginners to National Junior Champions, your division will be determined by your experience and wins at RECOGNIZED shows not your overall riding experience.  Do not be insulted by a placement in a "Novice" division.  Novice is the largest division in IHSA.
